THIAZIDE DIURETICS
Why this applies to Amiloride, Hydrochlorothiazide
Amiloride, Hydrochlorothiazide falls under THIAZIDE DIURETICS — the group Calcium interacts with.
Thiazides reduce calcium excretion by the kidneys. Using thiazides along with moderately large amounts of calcium carbonate increases the risk of milk-alkali syndrome (hypercalcemia, metabolic alkalosis, renal failure). Patients may need to have their serum calcium levels and/or parathyroid function monitored regularly.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here is the key thing to know: the hydrochlorothiazide part of your Moduretic actually tells your kidneys to hold onto calcium instead of flushing it out. So if you add a calcium supplement on top, that calcium can slowly build up in your blood. With larger doses (think antacids like Tums taken often, or high-dose calcium carbonate), this can lead to milk-alkali syndrome, which means too much calcium, an upset acid balance, and strain on the kidneys.
Watch for nausea, constipation, feeling unusually thirsty, confusion, or weakness. The everyday calcium in your food is unlikely to be a problem, but concentrated supplements are worth a conversation. Please don't stop either medicine on your own. Ask your pharmacist or doctor about the right calcium dose and whether your blood calcium should be checked.

This combination tablet is used in people with high blood pressure or congestive heart failure who develop low potassium (hypokalemia) when thiazides or similar diuretics are used alone.
It is also used when keeping potassium in the normal range is especially important, such as in people taking digitalis or those with significant heart rhythm problems. It can be used alone or added to other blood pressure medicines like methyldopa or beta blockers.
