Why this applies to Bendamustine

Bendamustine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Cannabidiol may increase levels of drugs metabolized by CYP1A2.
In vitro research shows that cannabidiol inhibits CYP1A2. Furthermore, clinical studies show that cannabidiol may inhibit the metabolism of caffeine, a CYP1A2 substrate. Two pharmacokinetic studies in healthy adults show that taking cannabidiol dosed 640 mg once up to 750 twice daily increases the area under the curve of caffeine. However, results are mixed over whether cannabidiol impacts peak serum levels of caffeine.

Here's the gist: bendamustine is partly cleared by a liver enzyme called CYP1A2, and CBD can slow that enzyme down. When that pathway is blocked, your body may break down bendamustine more slowly, so more of it can hang around in your bloodstream. Since bendamustine is a chemotherapy drug, even modest increases in its levels could mean stronger side effects, like low blood counts, more nausea, fatigue, or a higher chance of infection.

This concern comes mostly from lab work and from CBD's known effect on this enzyme, not from studies in people taking bendamustine, so it's a reasonable caution rather than a sure thing. Because cancer treatment is finely dosed, please don't start or stop CBD on your own. Loop in your oncology team or pharmacist first so they can guide you safely.

What Is Bendamustine Injection Used For?

Brand names: Treanda, Belrapzo, Vivimusta

Bendamustine hydrochloride injection is used to treat adults with chronic lymphocytic leukemia (CLL). It is also used for adults with indolent B-cell non-Hodgkin lymphoma (NHL) — a slow-growing type of blood cancer — when the disease has continued to progress during or within six months of treatment with rituximab or a rituximab-containing regimen.

For CLL, how well bendamustine works compared to other first-line treatments (aside from chlorambucil) has not been fully established.
