Cannabidiol (cbd) and Dextroamphetamine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Why this applies to Dextroamphetamine

Dextroamphetamine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cannabidiol (cbd) interacts with.

Theoretically, cannabidiol might increase levels of drugs metabolized by CYP2D6.
In vitro research shows that cannabidiol inhibits CYP2D6. Theoretically, concomitant use of cannabidiol with CYP2D6 substrates might increase the risk for adverse effects from these substrates. However, a clinical crossover trial in healthy adults shows that cannabidiol does not inhibit dextromethorphan, a substrate of CYP2D6.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the basic idea: your body uses an enzyme called CYP2D6 to help break down dextroamphetamine. In lab dish studies, CBD can slow that enzyme down. If that happened in real life, your dextroamphetamine could clear out a bit slower and build up, which might mean stronger stimulant effects like a faster or pounding heartbeat, higher blood pressure, jitteriness, trouble sleeping, or a poor appetite.

I want to be honest about how shaky this concern is. It's theoretical, based only on test-tube research. In a real human study, CBD did not meaningfully block CYP2D6 for a similar drug. So the worry is mostly on paper. Still, since CBD products vary a lot in quality and strength, it's worth a quick chat with your pharmacist or doctor before combining them.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.

From our pharmacist’s overview on the Dextroamphetamine page

“Dextroamphetamine (Dexedrine) is a stimulant for ADHD and narcolepsy, and I read this list with genuine wariness. Six of the 129 ingredients rate major, most of them because other stimulants, ephedra being the clearest, pile extra heart rate and blood pressure on top of what the drug already produces.”
