Preliminary clinical research, case reports, and animal studies suggest that high dose cannabidiol has sedative and hypnotic effects. Theoretically, concomitant use of cannabidiol with drugs with sedative and anesthetic properties may cause additive therapeutic and adverse effects.

This combination pill pairs hyoscyamine with phenobarbital, and it's the phenobarbital part that matters most here. Phenobarbital is a barbiturate that already slows down your brain to calm nerves and reduce stomach cramping. CBD, especially at higher doses, can have its own sedative effect. Put them together and those calming effects may stack up, so you could feel more drowsy, foggy, dizzy, or sluggish than usual.

To be fair, this concern is mostly theoretical, based on how these substances behave rather than strong studies. Still, since phenobarbital is a potent sedative, it's worth taking seriously. Don't change anything on your own. Check with your pharmacist or doctor before adding CBD, and be extra careful with driving until you know how you feel.

Here's the thing to keep in mind with this combo: it's the CBD that gets affected, not your medicine. The phenobarbital part of Levsin PB is a known liver enzyme booster (it speeds up CYP2C19), and CBD happens to be broken down by that same enzyme. So in theory, phenobarbital could cause your body to clear CBD faster, meaning you may get less benefit from the CBD than you expected.

I'll be honest, this is a theoretical concern based on how these drugs behave, not something proven in studies. Your hyoscyamine and phenobarbital should still work as usual. Since phenobarbital is a controlled medicine with real sedation and dependence concerns, please don't change anything on your own. Just loop in your pharmacist or doctor before adding CBD.

Here's the thing to know about this combination: the phenobarbital part of your medicine is what matters. Phenobarbital is a strong inducer, meaning it revs up a liver enzyme called CYP3A4. Since your body uses that same enzyme to break down CBD, taking phenobarbital can speed up how fast CBD is cleared out. The practical result is that your CBD may not work as well as you'd expect, since less of it sticks around.

This is a theoretical concern based on how these substances behave in the body, not something proven in studies, so I don't want to alarm you. It also doesn't make your phenobarbital or hyoscyamine more dangerous. Theoretically, cannabidiol might increase levels of drugs metabolized by CYP2B6.
In vitro research shows that cannabidiol inhibits CYP2B6. However, this interaction has yet to be reported in humans. Until more is known, use with caution. Theoretically, concomitant use of cannabidiol with CYP2B6 substrates might increase the risk for adverse effects from these substrates.

Here's the gist with this combo. Your medicine contains phenobarbital, a barbiturate that your liver clears partly through an enzyme called CYP2B6. In lab (test tube) studies, CBD appears to slow that same enzyme down. So in theory, adding CBD could let phenobarbital build up a bit more than usual, which might mean extra drowsiness, dizziness, grogginess, slowed breathing, or feeling unsteady on your feet. The hyoscyamine part isn't really the concern here.

I want to be honest about the strength of this: it's theoretical only and hasn't actually been reported in real people yet. Still, phenobarbital is a drug where small shifts can matter, so I'd play it safe. Please check with your pharmacist or doctor before pairing CBD with it.
