Rabeprazole Sodium falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cannabis interacts with.

Theoretically, cannabis may increase the levels and adverse effects of CYP3A4 substrates.
In vitro research shows that cannabis can inhibit the activity of CYP3A4 enzymes, which might decrease the metabolism of CYP3A4 substrates. In vitro research also shows that cannabis extracts modestly inhibit the CYP3A4 metabolism of testosterone; extracts providing the specific cannabinoids CBD and cannabigerol (CBG) had stronger inhibitory effects than extracts containing THC and CBD.

Here's the gist: rabeprazole (Aciphex) gets cleared from your body partly by a liver enzyme called CYP3A4. Lab studies suggest cannabis may slow that enzyme down a bit. In theory, that could let rabeprazole linger a little longer and build up slightly, which might nudge up side effects like headache, nausea, gas, or diarrhea.

The good news is this is mostly theory. The evidence comes from test-tube and animal studies, not real people, and rabeprazole is generally well tolerated with a wide safety margin. So I wouldn't lose sleep over it. Still, if you use cannabis regularly and notice new or stronger stomach symptoms, mention it to your pharmacist or doctor so they can take a look.

Research shows that cannabidiol (CBD), a constituent of cannabis, inhibits CYP2C19. In clinical studies and case reports, cannabidiol use resulted in significant increases in the serum levels of topiramate, methadone, citalopram, omeprazole, and N-desmethylclobazam, the primary active metabolite of clobazam. These chemicals are metabolized by CYP2C19. Concomitant use of cannabis with CYP2C19 substrates may increase the risk for adverse effects from these substrates.

Here's the simple version: your body uses an enzyme called CYP2C19 to clear rabeprazole (Aciphex) out of your system. The CBD in cannabis can slow that enzyme down. When the enzyme works slower, more rabeprazole can linger in your blood than usual. Rabeprazole is an acid-reducing medicine, not a prodrug, so higher levels just mean a bit more drug effect, which could nudge up common side effects like headache, nausea, diarrhea, or gas.

The good news is this is mostly a theoretical concern based on how these compounds behave, and rabeprazole is generally well tolerated. Still, if you use cannabis regularly and notice new stomach upset or headaches, mention it to your pharmacist or doctor before changing anything.

What Is Rabeprazole Used For?

Brand names: Aciphex, Aciphex Sprinkle

Rabeprazole delayed-release tablets treat a range of acid-related conditions in adults, including healing and maintenance of erosive or ulcerative GERD (gastroesophageal reflux disease), symptomatic GERD, duodenal ulcers, and pathological hypersecretory conditions such as Zollinger-Ellison syndrome. They are also used alongside antibiotics (amoxicillin and clarithromycin) to eradicate H. pylori bacteria and reduce the risk of duodenal ulcer coming back.

In adolescents 12 years and older, rabeprazole delayed-release tablets are approved for short-term treatment of symptomatic GERD. Both AcipHex and Rabeprazole Sodium delayed-release tablets share this indication set for adults and adolescents 12 and older.
