Hydrocodone, Ibuprofen falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Cinchona interacts with.

Theoretically, taking cinchona might inhibit cytochrome P450 2D6 (CYP2D6) and increase levels of drugs metabolized by this enzyme.
Quinidine, a constituent of cinchona, inhibits CYP2D6-mediated metabolism in humans.

Here's the wrinkle with Vicoprofen (hydrocodone plus ibuprofen). The hydrocodone part is a prodrug in one sense: your body uses an enzyme called CYP2D6 to convert some of it into hydromorphone, a stronger pain-relieving form. Cinchona bark contains quinidine, which can block that same enzyme. So in theory, less of your hydrocodone gets turned into its more potent form, which could mean weaker pain relief than expected.

I'll be honest, this is based on pharmacology theory, not strong real-world studies, so for most people it may not be dramatic. But cinchona bark itself is unpredictable and carries real heart risks. Before combining them, please check with your pharmacist or doctor first.

Theoretically, taking cinchona might increase the drug effects and risk of bleeding with anticoagulant and antiplatelet drugs.
In vitro evidence shows that the alkaloids quinine, quinidine, and cinchonine, constituents of cinchona, inhibit platelet aggregation.

Here's what's going on with Vicoprofen (a combo of hydrocodone for pain and ibuprofen, an NSAID). The bleeding concern here ties to the ibuprofen piece. Ibuprofen already nudges your platelets toward not sticking together well, and it can irritate the stomach lining. Cinchona's natural compounds (quinine, quinidine, cinchonine) have shown, in lab dish studies only, that they can also keep platelets from clumping. Stack those two together and, in theory, your blood may be a little slower to clot.

This is a theoretical concern based on test-tube data, not strong human evidence, so don't panic. Still, watch for easy bruising, nosebleeds, or stomach upset and black stools. Loop in your pharmacist or doctor before pairing these.
