Alpelisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Corydalis Yanhusuo interacts with.

Theoretically, Corydalis yanhusuo might increase serum levels of drugs metabolized by CYP3A4.
Corydalis yanhusuo contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, Corydalis yanhusuo might also inhibit CYP3A4.

Here's the gist for Piqray (alpelisib): your liver uses an enzyme called CYP3A4 to help clear this drug out of your body. Corydalis yanhusuo contains berberine, which lab and early human studies suggest can mildly slow that same enzyme down. If it does, alpelisib could clear a bit more slowly and build up in your blood, which may mean stronger effects and a higher chance of side effects like high blood sugar, rash, diarrhea, nausea, or mouth sores.

I want to be honest though: this is a theoretical concern based on how these compounds behave, not proven harm in people taking them together. Since alpelisib is a cancer medicine that needs careful dosing, please don't start or stop anything on your own. Just run it by your oncologist or pharmacist first.

About Alpelisib

What Is Alpelisib Used For?

Brand name: Piqray

Alpelisib is approved for two distinct uses, each tied to a specific brand. Piqray is used in combination with another drug called fulvestrant to treat adults with hormone receptor (HR)-positive, HER2-negative, advanced or metastatic breast cancer whose tumors carry a PIK3CA mutation — and only after the cancer has progressed on or after a hormone-based treatment. A companion diagnostic test is required before starting Piqray.

Vijoice is used to treat adults and children 2 years of age and older who have severe manifestations of PIK3CA-Related Overgrowth Spectrum (PROS) — a rare condition where PIK3CA gene mutations cause abnormal tissue, vascular, and bone overgrowth — and who need systemic (whole-body) therapy. Vijoice's approval for PROS was granted under the FDA's accelerated approval pathway based on response data, and continued approval may depend on confirmatory trials.
