Pirfenidone falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Diindolylmethane interacts with.

Theoretically, diindolylmethane might lower serum levels of CYP1A2 substrates.
In vitro evidence suggests that diindolylmethane can induce CYP1A2. Theoretically, it might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.

Here's the gist for Esbriet (pirfenidone): your liver clears it using an enzyme called CYP1A2. Lab studies hint that DIM may rev up that enzyme. If that happened in real life, your body could break pirfenidone down a little faster, meaning slightly lower blood levels and possibly a bit less protection for your lungs.

I want to be honest, though, this is only a theoretical concern from test-tube research. It has never actually been reported in people, and it's rated minor and unlikely. So I wouldn't lose sleep over it. Since pirfenidone is treating something as serious as pulmonary fibrosis, just loop in your pharmacist or doctor before adding DIM so they can keep an eye on things.

Pirfenidone — available as capsules and tablets under the brand name Esbriet, as well as generic pirfenidone products — is indicated for the treatment of idiopathic pulmonary fibrosis (IPF), a chronic, progressive disease in which scar tissue builds up in the lungs and makes breathing increasingly difficult.

Esbriet capsules and tablets, pirfenidone capsules, and pirfenidone tablets are each specifically approved for IPF. No other uses are supported by the available product labels.
