Amiloride, Hydrochlorothiazide falls under DIURETIC DRUGS — the group Diindolylmethane interacts with.

Theoretically, diindolylmethane might increase the risk of hyponatremia if used with sodium-depleting diuretics.
Large doses of diindolylmethane (600 mg daily) have been associated with two cases of asymptomatic hyponatremia in clinical research.

Here's the gist: your water pill, the amiloride and hydrochlorothiazide combo, already nudges your body to flush out sodium along with extra fluid. The hydrochlorothiazide part especially can lower your sodium level. DIM may add a little to that effect. In a small amount of clinical research, fairly large doses of DIM (around 600 mg a day) were linked to two cases of low sodium that didn't cause symptoms.

This is still mostly theoretical, so I wouldn't panic. But if sodium drops too far, you can feel tired, dizzy, confused, headachy, or crampy. Please don't start or stop anything on your own. Loop in your pharmacist or doctor before adding DIM, and they may simply check a basic sodium level to be safe.

Although early lab studies look interesting, strong human evidence is lacking for most of these uses. What Is Amiloride and Hydrochlorothiazide Used For?

Brand names: Moduretic, Amilzide, Moduret 25, Amil-Co

Amiloride hydrochloride and hydrochlorothiazide tablets are used for patients with high blood pressure (hypertension) or congestive heart failure who tend to develop low potassium (hypokalemia) when taking a thiazide or other diuretic alone — or for whom keeping potassium levels in a normal range is especially important, such as people taking digoxin or those with significant heart rhythm problems.

The tablets may be used on their own or alongside other blood pressure medicines, such as methyldopa or beta blockers. This combination product is not intended as the first-choice treatment for swelling (edema) or high blood pressure unless low potassium is a specific concern.
