Perampanel falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Diindolylmethane interacts with.

Theoretically, diindolylmethane might lower serum levels of CYP1A2 substrates.
In vitro evidence suggests that diindolylmethane can induce CYP1A2. Theoretically, it might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.

Here's the gist for perampanel (Fycompa). Your liver helps clear perampanel, partly through an enzyme called CYP1A2. Lab studies hint that DIM may speed that enzyme up. In theory, a faster enzyme could clear perampanel a little quicker and nudge its blood levels down. For a seizure medicine, the worry would be slightly weaker seizure control rather than more side effects.

I want to be honest about how soft this is. This comes only from test-tube data and has never actually been reported in people, so it's rated minor and unlikely. Still, since missing seizure protection matters, it's worth a quick chat with your neurologist or pharmacist before adding DIM. Don't change your perampanel on your own.

Although early lab studies look interesting, strong human evidence is lacking for most of these uses. What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
