BISPHOSPHONATES
Why this applies to Tiludronate Disodium
Tiludronate Disodium falls under BISPHOSPHONATES — the group Dolomite interacts with.
Cations, including calcium and magnesium, can decrease absorption of bisphosphonates. Advise patients to take bisphosphonates at least 30 minutes before dolomite, but preferably at a different time of day.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the key thing with Skelid (tiludronate): your body only absorbs a small fraction of each dose to begin with, and that little bit is easily blocked. The calcium and magnesium in dolomite can bind to the tiludronate right there in your stomach, so even less gets into your bloodstream. The worry isn't a dangerous reaction, it's the opposite: your Paget's treatment may simply not work as well as it should.
This is based on how these minerals behave rather than hard studies, but it's a sensible concern. The simple fix is timing. Take your tiludronate on an empty stomach with plain water, then wait at least 2 hours before any dolomite, calcium, or magnesium. Honestly, given dolomite can carry traces of lead and other metals, ask your pharmacist about a cleaner calcium source too.
