Dusty Miller and Phenobarbital, Phenytoin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) INDUCERS

Why this applies to Phenobarbital, Phenytoin

Phenobarbital, Phenytoin falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Dusty Miller interacts with.

Theoretically, CYP3A4 inducers might increase the risk of adverse effects from the pyrrolizidine alkaloid (PA) constituents in dusty miller.
Dusty miller contains PAs. PAs are substrates of CYP3A4. Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to understand with this pairing. Both phenobarbital and phenytoin are strong inducers, meaning they rev up the liver enzyme (CYP3A4) that processes a lot of substances, including the pyrrolizidine alkaloids found in Dusty Miller. The worry isn't about your seizure medicine getting weaker. Instead, by speeding up that enzyme, your medication could push the body to convert more of those plant compounds into toxic byproducts that can stress the liver.

I want to be honest: this is a theoretical concern based on lab thinking, not real patient studies, so don't be alarmed. But Dusty Miller isn't considered safe to take anyway. Please check with your pharmacist or doctor before using any product made from it.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

About Dusty Miller

What is Dusty Miller?

  • Cataracts (traditional eye use)
  • Eye irritation and inflammation
  • Wound healing (folk use)
  • Menstrual complaints (folk use)

Dusty Miller is a silvery ornamental plant that has been used in folk medicine, mainly as an eye remedy for cataracts and irritation. There is no reliable scientific evidence that it works, and the plant contains compounds (pyrrolizidine alkaloids) that can be toxic, so it is not considered safe for self-treatment. Talk to a pharmacist or doctor before using any product made from it.
