The piece of this combo cough-and-cold product that matters here is the dextromethorphan. It nudges serotonin activity in the brain, and so does fermented wheat germ extract, at least in animal studies. Put the two together and, in theory, you could stack up too much serotonin. That can show up as things like restlessness, a racing heart, sweating, shivering, muscle twitching, or feeling agitated.

Honestly, I want to keep this in perspective. The concern comes from lab and animal research only, not from people, so it is more of a caution than a proven problem. Still, since pseudoephedrine already revs you up a bit, it is worth a quick chat with your pharmacist or doctor before pairing these, especially if you take other serotonin-affecting medicines.
