Dextromethorphan, Guaifenesin, Pseudoephedrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Fever Bark interacts with.

Theoretically, fever bark might increase the levels and clinical effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of fever bark, inhibits CYP2D6 enzyme activity. The effects of fever bark itself are unclear.

Here's the simple version. One ingredient in your cough and cold combo, the dextromethorphan, is cleared from your body mainly by a liver enzyme called CYP2D6. Fever bark contains a compound (yohimbine) that may slow that same enzyme down. If it does, dextromethorphan could build up a bit and you might feel more of its effects, things like drowsiness, dizziness, or feeling foggy and lightheaded.

Honestly though, this is only a theoretical concern based on lab studies, not real people, so we don't know if fever bark actually does this in the body. I'd be more cautious about fever bark's own effects on blood pressure and heart rate stacking with the pseudoephedrine here, which can already raise both. Since fever bark has potent alkaloids and little safety data, please check with your pharmacist or doctor before combining them.

Theoretically, fever bark might increase or decrease the effects of stimulant drugs.
Reserpine, a constituent of fever bark, might increase or decrease effects of sympathomimetics. Also, yohimbine, another constituent of fever bark, has sympathomimetic effects and increases blood pressure in a dose-dependent manner. It is not known if this would occur with fever bark.

The part of this combo product to watch here is the pseudoephedrine, which is a stimulant decongestant. Fever bark contains active plant alkaloids, including reserpine-like and yohimbine-like compounds, that can tug on your blood pressure and heart rate. On paper, mixing those with pseudoephedrine could push things in an unpredictable direction, possibly stacking up effects like a racing or pounding heart, jitteriness, higher blood pressure, or trouble sleeping. In some cases the reserpine-like piece might actually blunt the decongestant instead.

Honestly, this is mostly theory based on a few of the bark's chemicals, not solid human studies on fever bark itself. Still, because the direction could go either way, I'd skip fever bark or run it by your pharmacist or doctor first, especially if you have any heart or blood pressure issues.

Fever bark is the bark of the Australian tree Alstonia constricta, used in traditional medicine for fever, diarrhea, and infections. There is very little reliable human research to support these uses, and the bark contains potent alkaloids that may cause serious side effects. It is best avoided unless used under direct medical supervision.
