Dextromethorphan HBr, Phenylephrine HCl falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Fever Bark interacts with.

Theoretically, fever bark might increase the levels and clinical effects of CYP2D6 substrates.
In vitro research suggests that yohimbine, a constituent of fever bark, inhibits CYP2D6 enzyme activity. The effects of fever bark itself are unclear.

Here's the gist with your Vicks Formula 44D. The cough part, dextromethorphan, gets cleared from your body by a liver enzyme called CYP2D6. Fever bark contains a yohimbine-like compound that may slow that enzyme down, at least in lab tests. If it does, dextromethorphan could build up a bit, which can mean more drowsiness, dizziness, or feeling foggy. The phenylephrine isn't really the concern here, but remember fever bark's alkaloids can themselves nudge blood pressure and heart rate around.

I'll be straight with you: this is theoretical and based only on test-tube work, so the real-world effect from fever bark is unclear. Still, since fever bark is risky on its own, I'd run this combination past your pharmacist or doctor before mixing them.

Theoretically, fever bark might increase or decrease the effects of stimulant drugs.
Reserpine, a constituent of fever bark, might increase or decrease effects of sympathomimetics. Also, yohimbine, another constituent of fever bark, has sympathomimetic effects and increases blood pressure in a dose-dependent manner. It is not known if this would occur with fever bark.

The part of Vicks Formula 44D I'd watch here is the phenylephrine, a decongestant that works by tightening blood vessels, which can nudge your blood pressure and heart rate up. Fever bark carries plant alkaloids, including yohimbine-like and reserpine-like compounds, that also tug on those same blood-pressure and nervous-system controls. Stacking the two could, in theory, make those effects unpredictable, either adding to the pressure rise or working against it.

Honestly, this is built on weak, anecdotal evidence, and we don't even know how much active alkaloid a given fever bark dose delivers. The dextromethorphan (cough suppressant) side isn't really the concern. Still, since fever bark itself is risky and poorly studied, I'd skip it and check with your pharmacist or doctor first.

Fever bark is the bark of the Australian tree Alstonia constricta, used in traditional medicine for fever, diarrhea, and infections. There is very little reliable human research to support these uses, and the bark contains potent alkaloids that may cause serious side effects. It is best avoided unless used under direct medical supervision.
