Fever Bark and Ibuprofen, Pseudoephedrine

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Fever Bark interacts with whole categories of medication. Ibuprofen, Pseudoephedrine falls into 2 of those categories, so it can interact with Fever Bark in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

STIMULANT DRUGS

1 of 2

Why this applies to Ibuprofen, Pseudoephedrine

Ibuprofen, Pseudoephedrine falls under STIMULANT DRUGS — the group Fever Bark interacts with.

Theoretically, fever bark might increase or decrease the effects of stimulant drugs.
Reserpine, a constituent of fever bark, might increase or decrease effects of sympathomimetics. Also, yohimbine, another constituent of fever bark, has sympathomimetic effects and increases blood pressure in a dose-dependent manner. It is not known if this would occur with fever bark.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the part that matters for you: the pseudoephedrine in this Dristan product is a stimulant (a sympathomimetic), and fever bark carries plant alkaloids that pull in different directions. One of them, yohimbine, can act like a stimulant itself and nudge blood pressure up, while another, reserpine, can sometimes blunt or alter how stimulants behave. Because of that tug-of-war, the combo could raise your blood pressure, speed your heart, or leave you jittery, restless, or unable to sleep. The ibuprofen side can also raise blood pressure a bit on its own.

Honestly, this is mostly theoretical and based on weak, anecdotal reports, so I wouldn't panic. Still, fever bark is potent and poorly studied, so please check with your pharmacist or doctor before pairing them, especially if you have any heart or blood pressure issues.

ANTICOAGULANT/ANTIPLATELET DRUGS

2 of 2

Ibuprofen, Pseudoephedrine also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

Theoretically, combining fever bark with antiplatelet or anticoagulant drugs might have additive effects; however, this has not been reported in clinical research.
Research in healthy adults shows that taking yohimbine, a constituent of fever bark, in doses of 8 mg or more, seems to inhibit platelet aggregation in vitro by binding to the alpha-2 adrenoceptor. The effects of fever bark itself are unclear.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with Dristan (which combines ibuprofen and pseudoephedrine) and fever bark. The concern is mostly about the ibuprofen side. Ibuprofen already nudges your platelets toward being a little less sticky, so it can slightly raise your risk of bruising or bleeding. Some lab studies suggest yohimbine, one of the alkaloids in fever bark, may also make platelets clump less. Put together, the theory is they could add up.

I want to be honest, though: this is rated minor and rests only on test-tube and animal data, with nothing seen in real patients. There's also a separate wrinkle, fever bark's alkaloids can affect blood pressure and heart rate, and pseudoephedrine raises those too. Please run it by your pharmacist or doctor before combining them.

About Fever Bark

What is Fever Bark?

Fever Bark
  • Fever
  • Diarrhea
  • Malaria (traditional)
  • Digestive complaints
  • Bitter tonic

Fever bark is the bark of the Australian tree Alstonia constricta, used in traditional medicine for fever, diarrhea, and infections. There is very little reliable human research to support these uses, and the bark contains potent alkaloids that may cause serious side effects. It is best avoided unless used under direct medical supervision.
