Why this applies to Apalutamide

Apalutamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goji interacts with.

Theoretically, goji berry might inhibit CYP3A4 and reduce metabolism of CYP3A4 substrates.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.

Here's the basic idea with goji and Erleada (apalutamide). Your body uses a liver enzyme called CYP3A4 to help break down apalutamide. Lab studies suggest goji juice might slow that enzyme down a bit. If that happened in people, apalutamide could clear more slowly and build up, which could mean stronger effects and a higher chance of side effects like fatigue, joint pain, rash, high blood pressure, or diarrhea.

The honest part: this is only a theoretical concern from test-tube research, and nothing like it has actually been reported in real patients. Eating goji berries as a normal food is unlikely to cause trouble. I'd be more cautious with concentrated goji supplements or large amounts of juice. Either way, it's worth a quick check-in with your oncology pharmacist or doctor before starting them.

About Apalutamide

What Is Apalutamide Used For?

Brand name: Erleada

Erleada (apalutamide) is used to treat two types of prostate cancer. The first is metastatic castration-sensitive prostate cancer (mCSPC) — prostate cancer that has spread to other parts of the body and still responds to hormone-lowering therapy. The second is non-metastatic castration-resistant prostate cancer (nmCRPC) — prostate cancer that has stopped responding to standard hormone therapy but has not yet spread to other organs.

Erleada is always used alongside a gonadotropin-releasing hormone (GnRH) analog — a medicine that lowers testosterone — or in patients who have already had surgery to remove the testicles.
