Dextromethorphan, Guaifenesin brand names: Sudal DM, Touro DM, Queltuss, Fenesin DM, Monafed DM, and others.
Dextromethorphan, Guaifenesin falls into 2 drug categories that Goji can interact with; the most serious is rated moderate. Theoretically, goji berry might inhibit CYP2D6 and reduce metabolism of CYP2D6 substrates.

These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Goji interacts with whole categories of medication. Dextromethorphan, Guaifenesin falls into 2 of those categories, so it can interact with Goji in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
1 of 2Why this applies to Dextromethorphan, Guaifenesin
Dextromethorphan, Guaifenesin falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Goji interacts with.
In vitro research shows that goji berry juice inhibits CYP2D6 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP2D6 substrates. However, this has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this combo. The cough suppressant part, dextromethorphan, gets cleared from your body by a liver enzyme called CYP2D6. In lab dishes, goji berry juice has been shown to slow that same enzyme down. If that held true in people, your body might clear dextromethorphan a little slower, letting it build up. That could mean more drowsiness, dizziness, or a foggy, lightheaded feeling. The guaifenesin (the mucus thinner) isn't really affected here.
The reassuring part: this is only test-tube evidence and has never actually been reported in real patients. Eating goji berries as a food is very unlikely to cause trouble. Still, if you're using concentrated goji supplements, it's worth a quick chat with your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Dextromethorphan, Guaifenesin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version on combining goji with a cough syrup like Sudal DM. Dextromethorphan (the cough suppressant) is partly cleared by a liver enzyme called CYP3A4. In lab studies, goji berry juice appears to slow that enzyme down. If that happened in real life, dextromethorphan could build up a bit, which might mean more drowsiness, dizziness, or feeling foggy. The guaifenesin part isn't really affected this way.
The key thing: this is only test-tube evidence and hasn't actually shown up in people. Eating goji berries as food is very unlikely to cause trouble. If you're using strong, concentrated goji supplements, just run it by your pharmacist or doctor first to be safe.

What These Ratings Mean
Every interaction above is graded three ways — here’s the key to each label.
Clinically significant. Avoid the combination, or use only under close professional supervision.
Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.
Generally not serious. Be aware of it and mention it to your pharmacist.
- LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
- AHigh-quality meta-analysis (quantitative systematic review).
- BCase-control study.
- CConsensus.

From our pharmacist’s overview on the Dextromethorphan, Guaifenesin page
“Dextromethorphan with guaifenesin (Sudal DM, Touro DM, Queltuss, and others) sounds too ordinary a cough medicine to tangle badly with supplements, yet 4 of the 171 ingredients on this list rate major and belong at the top of your reading. Grapefruit and St.”
