Why this applies to Irinotecan
Irinotecan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goji interacts with.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for irinotecan (Camptosar): your liver uses an enzyme called CYP3A4 to help process this chemo drug. In lab studies, goji berry juice has been shown to slow that same enzyme down. If that happened in a real person, irinotecan could clear out more slowly and build up to higher levels, which could mean stronger side effects like severe diarrhea, low blood counts, nausea, and feeling wiped out.
I want to be honest about the evidence though: this is only seen in test-tube research, not in actual patients, so it's more of a theoretical caution than a proven problem. Still, because irinotecan is potent chemo with serious side effects, please check with your oncology team or pharmacist before using concentrated goji supplements. Eating a normal amount of the berries is far less likely to matter.
