Why this applies to Ketoconazole
Ketoconazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goji interacts with.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with goji and oral ketoconazole. Your body relies on a liver enzyme called CYP3A4 to break down ketoconazole and clear it out. Lab studies hint that goji might slow that enzyme down a bit. If that happened in real life, ketoconazole could build up a little more than usual, which can nudge up the chance of side effects like nausea, stomach upset, headache, or changes in your liver enzymes.
The important part: this is only seen in test-tube research, not in actual people, so it's very theoretical. A serving of goji berries as food is unlikely to cause trouble. Concentrated goji extracts or large amounts of juice are the bigger question mark, so check with your pharmacist or doctor before pairing them.

What Is Ketoconazole Used For?

Ketoconazole 2% shampoo and foam are used to treat tinea versicolor (also called pityriasis versicolor) — a fungal infection caused by Pityrosporum orbiculare (also known as Malassezia furfur), a yeast that naturally lives on the skin.
This infection creates light or dark patches, most often on the trunk, neck, arms, and upper thighs. Even after the fungus is cleared, those skin patches may take months to return to their normal color — that's normal, and depends on your skin type and sun exposure. Tinea versicolor can also come back, since the fungus lives on healthy skin even when there's no active infection.
