Goji and Mavacamten

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Goji interacts with whole categories of medication. Mavacamten falls into 2 of those categories, so it can interact with Goji in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

1 of 2

Why this applies to Mavacamten

Mavacamten falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Goji interacts with.

Theoretically, goji berry might inhibit CYP2C19 and reduce metabolism of CYP2C19 substrates.
In vitro research shows that goji berry tincture and juice inhibit CYP2C19 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP2C19 substrates. However, this has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the gist: your body relies heavily on a liver enzyme called CYP2C19 to clear mavacamten (Camzyos) out of your system. Lab studies suggest goji berry may slow that enzyme down a bit. If it does, your mavacamten could build up higher than expected. Since this medicine works by easing how forcefully your heart muscle squeezes, too much can push things too far, leading to a weaker heart pump, shortness of breath, dizziness, swelling, or fatigue. That's why it's a closely monitored drug with regular echocardiograms.

The reassuring part: this is only seen in test-tube research, not in real people, so the actual risk is uncertain. Still, given how carefully mavacamten is dosed, I'd run any concentrated goji supplement past your cardiologist or pharmacist first. Eating a few goji berries as food is far less of a concern.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Mavacamten also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, goji berry might inhibit CYP3A4 and reduce metabolism of CYP3A4 substrates.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.

Here's the situation with Camzyos (mavacamten) and goji. Your body relies on a liver enzyme called CYP3A4 to clear mavacamten out of your system. In lab studies, goji juice seems to slow that same enzyme down. In theory, that could let mavacamten build up higher than expected, which matters because this is a sensitive heart medicine with careful dosing.

If levels climbed too high, you might see more of its known effects, like a weaker heart squeeze (reduced ejection fraction), dizziness, shortness of breath, or swelling. That said, this is only a test-tube finding and hasn't been seen in real people, so it's a caution, not a proven problem. A goji berry snack is unlikely to matter, but concentrated supplements could. Please check with your pharmacist or cardiologist before mixing them.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)

From our pharmacist’s overview on the Mavacamten page

“Grapefruit is the first thing I would set aside for anyone taking mavacamten (Camzyos), the medicine for obstructive hypertrophic cardiomyopathy. It blocks CYP3A4, the liver enzyme that clears mavacamten, and too much of this drug can weaken the heart's pumping strength, the very problem careful dosing exists to prevent.”

About Mavacamten

What Is Mavacamten Used For?

Brand name: Camzyos

Camzyos (mavacamten) is approved for adults with symptomatic obstructive hypertrophic cardiomyopathy (HCM) — a condition where the heart muscle grows too thick and blocks blood flow out of the heart. Specifically, it is used in people whose symptoms fall into New York Heart Association (NYHA) class II or III, meaning they have noticeable limitations on physical activity.

The goal of treatment is to improve how much the patient can do physically and to reduce the symptoms that HCM causes, such as shortness of breath, chest pain, and fatigue.
