CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Thiotepa
Thiotepa falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goji interacts with.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing with thiotepa: it's a strong chemotherapy drug given for cancers like ovarian, breast, and bladder cancer, and your liver's CYP3A4 enzyme plays a role in handling it. Goji berry has been shown in lab dish studies (not in real people) to slow down that same enzyme. In theory, if goji slowed your body's clearance of thiotepa, levels could creep up and bring on more of the side effects you'd already be watching for, like low blood counts, infection risk, nausea, or mouth sores.
I'll be honest, this is a theoretical concern only, and small food amounts of goji berries are very unlikely to matter. Still, because chemo is serious business, please run any concentrated goji supplement past your oncology team or pharmacist before adding it.

About Thiotepa Injection
What Is Thiotepa Injection Used For?
Brand names: Thioplex, Tepadina
Tepadina (thiotepa) injection is used for several purposes. It treats adenocarcinoma (a type of cancer) of the breast or ovary. It is also used to control malignant effusions — abnormal fluid buildup in body cavities caused by cancer spreading to those areas. For bladder cancer, Tepadina treats superficial papillary carcinoma of the urinary bladder.
Tepadina is also used in pediatric patients with class 3 beta-thalassemia (a severe inherited blood disorder). In those patients, it is given as part of a preparative regimen — a conditioning treatment to reduce the risk of graft rejection before an allogeneic stem cell transplant (a transplant using donor stem cells), alongside the chemotherapy drugs busulfan and cyclophosphamide.
