Why this applies to Clopidogrel

Clopidogrel falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Goji interacts with.

Theoretically, goji berry might inhibit CYP2C9 and reduce metabolism of CYP2C9 substrates.
In vitro research shows that goji berry tincture and juice inhibit CYP2C9 enzymes. Additionally, multiple case reports suggest that goji berry concentrated tea and juice inhibit the metabolism of warfarin, a CYP2C9 substrate. Concomitant use with goji may decrease metabolism and increase levels of CYP2C9 substrates.

Here's the wrinkle with clopidogrel (Plavix): it's a prodrug, meaning it shows up inactive and your body has to switch it on. That activation mostly runs through a liver enzyme called CYP2C19, with a smaller role for CYP2C9. The goji interaction on file is about goji possibly slowing CYP2C9, and that signal comes mainly from lab studies and warfarin case reports, so it's pretty theoretical for clopidogrel.

If anything, slowing an enzyme that helps turn clopidogrel into its active form could mean slightly less clot protection, not more. That's mostly speculation here, and food amounts of goji are unlikely to matter. Still, since clopidogrel guards against heart attack and stroke, check with your pharmacist before using concentrated goji juice, tea, or capsules.

Theoretically, goji berry might inhibit CYP3A4 and reduce metabolism of CYP3A4 substrates.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.

Here's the thing about Plavix (clopidogrel): it's a prodrug, meaning it arrives in your body inactive and your liver enzymes have to switch it on before it can thin your blood and protect against clots. The main enzyme that activates it is CYP2C19, not CYP3A4, so the lab finding here (goji possibly slowing CYP3A4) doesn't line up neatly with how clopidogrel actually works.

On top of that, this is only a test-tube observation. It has never been shown to happen in real people, and the small amount of goji in food is very unlikely to matter. Because clopidogrel is a drug you really don't want weakened, I'd still mention concentrated goji supplements to your pharmacist or doctor just to be safe, but there's no reason to panic over a few berries.

What Is Clopidogrel Used For?

Brand name: Plavix

Clopidogrel tablets are used to reduce the risk of heart attack and stroke in people with acute coronary syndrome (ACS) — including unstable angina and non-ST-elevation or ST-elevation heart attacks — and in people with a history of recent heart attack, recent stroke, or established peripheral arterial disease. For ACS, it is typically given alongside aspirin.

It works by preventing platelets (blood cells that trigger clotting) from clumping together, which helps keep arteries open and lowers the chance of a dangerous clot forming.
