Goji and Abemaciclib

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Abemaciclib

Abemaciclib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goji interacts with.

Theoretically, goji berry might inhibit CYP3A4 and reduce metabolism of CYP3A4 substrates.
In vitro research shows that goji berry juice inhibits CYP3A4 enzymes. Concomitant use with goji may decrease metabolism and increase levels of CYP3A4 substrates. However, this has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Your body clears abemaciclib (Verzenio) mainly through a liver enzyme called CYP3A4. In lab studies, goji berry juice slowed that same enzyme down. If that happened in a real person, abemaciclib could be broken down more slowly and build up to higher levels than expected, which might mean more of its usual side effects like diarrhea, fatigue, nausea, low blood counts, or infections.

Here's the reassuring part: this is only test-tube evidence and has not been seen in actual patients. Everyday food amounts of goji berries are unlikely to cause trouble, but concentrated goji supplements or large amounts of juice are a bigger question mark. Since this is a cancer medication, please don't start or stop anything on your own. Just run your goji use past your oncologist or pharmacist first.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.

From our pharmacist’s overview on the Abemaciclib page

“A daily pill for a common form of advanced breast cancer, abemaciclib (Verzenio) only helps when its blood level stays steady, so this list of 138 supplements deserves genuine care. Nearly all of them act on CYP3A4, the liver enzyme that clears abemaciclib, meaning a supplement can push its level up and intensify side effects, or drag…”

About Abemaciclib

What Is Abemaciclib Used For?

Brand name: Verzenio

Verzenio (abemaciclib) is used to treat HR-positive, HER2-negative breast cancer in adults. For early breast cancer at high risk of recurrence, Verzenio is taken alongside endocrine therapy (tamoxifen or an aromatase inhibitor) after surgery to help prevent the cancer from coming back.

For advanced or metastatic breast cancer (cancer that has spread), Verzenio can be used in combination with an aromatase inhibitor as a first hormonal treatment, in combination with fulvestrant after the cancer has progressed on endocrine therapy, or alone after prior endocrine therapy and chemotherapy have stopped working.
