Why this applies to ADO-Trastuzumab Emtansine
ADO-Trastuzumab Emtansine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Green Tea interacts with.
In vitro and in vivo research suggests that green tea can inhibit intestinal CYP3A and induce hepatic CYP3A4 enzymes. However, this effect is unlikely to be clinically significant, as green tea does not appear to affect CYP3A4 activity in humans.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the good news on this pairing: the concern with green tea and CYP3A4 is mostly theoretical. Lab studies hint that green tea might nudge this enzyme system one way or the other, but in actual people it doesn't seem to meaningfully change how CYP3A4 drugs behave. The part of Kadcyla that gets cleared by CYP3A4 is its cancer-killing payload (DM1). In theory, slowing that breakdown could raise side effects like low platelets, liver enzyme changes, nerve tingling, or fatigue, but there's no good human evidence this actually happens with green tea.
For a serious cancer treatment like this, I'd still keep your oncology team in the loop about any supplements, especially concentrated green tea extracts, which can stress the liver on their own. Normal tea-drinking amounts are very unlikely to be an issue.

Kadcyla is used by itself (as a single agent) for HER2-positive breast cancer in two situations. Your tumor must test positive for HER2 using an FDA-authorized test.
- Metastatic breast cancer: for people who already received trastuzumab and a taxane, and who either had treatment for metastatic disease or whose cancer returned during or within six months of finishing adjuvant (after-surgery) therapy.
- Early breast cancer: as adjuvant treatment for people who still have residual invasive disease after neoadjuvant (pre-surgery) taxane and trastuzumab-based treatment.
