P-GLYCOPROTEIN SUBSTRATES

Darunavir, Ritonavir falls under P-GLYCOPROTEIN SUBSTRATES — the group Green Tea interacts with.
In vitro research and case reports suggest that green tea inhibits drug efflux by P-gp, potentially increasing serum levels of P-gp substrates. Case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking green tea and certain P-gp substrates.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with your HIV combo of darunavir and ritonavir. Both of these drugs get moved around your body by a little cellular pump called P-glycoprotein (P-gp). Green tea, in concentrated amounts, may partly block that pump. If it does, a bit more of the medication could hang around in your bloodstream, which in theory could nudge up side effects like nausea, diarrhea, headache, or an upset stomach.
Now, I want to be honest about the evidence here: this is anecdotal, based mostly on lab studies and scattered case reports, not solid human trials. A cup or two of brewed green tea is unlikely to cause trouble. Concentrated green tea extract pills are where I'd be more careful. Since keeping HIV medication levels steady really matters, please run any green tea supplement past your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Darunavir, Ritonavir also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro and in vivo research suggests that green tea can inhibit intestinal CYP3A and induce hepatic CYP3A4 enzymes. However, this effect is unlikely to be clinically significant, as green tea does not appear to affect CYP3A4 activity in humans.

Great question. Both darunavir and ritonavir are handled by a liver and gut enzyme called CYP3A4, and green tea has shown some mixed effects on that enzyme in lab and animal studies. On paper it could slow the enzyme in your gut (nudging levels up) or speed it up in your liver (nudging levels down), but here's the key point: in actual people, green tea does not appear to meaningfully change CYP3A4 activity.
So drinking green tea as a normal beverage is very unlikely to change how well your HIV medicine works or cause extra side effects like nausea or diarrhea. This is rated minor and rests on weak lab-only evidence. Still, concentrated green tea extract pills are a different animal, so run those by your pharmacist or doctor before starting.
