Here's the gist with your birth control pill (the ethinyl estradiol and norgestrel combo) and green tea. Drinking green tea as a beverage isn't the concern here. The worry is with concentrated green tea extract supplements, which in rare cases have been tied to liver irritation. Since estrogen-containing birth control pills can also occasionally stress the liver, the theory is that combining the two could add up and put a little extra strain on it.

I'll be honest: this is based on scattered case reports, not strong proof, and it's considered unlikely. Still, if you take a green tea extract supplement, watch for signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and check with your pharmacist or doctor before starting one.

CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS

2 of 6

Ethinyl Estradiol, Norgestrel also falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS.

Theoretically, concomitant use might increase the levels and adverse effects of caffeine.
Green tea contains caffeine. Caffeine is metabolized by cytochrome P450 1A2 (CYP1A2),. Theoretically, drugs that inhibit CYP1A2 may decrease the clearance rate of caffeine from green tea and increase caffeine levels.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing many people don't realize: your birth control pill (the estrogen part, ethinyl estradiol) can gently slow down how your body clears caffeine. Since green tea naturally contains caffeine, drinking it while on Ovral or Lo/Ovral could mean the caffeine lingers a bit longer than usual.

In real life, that might show up as feeling jittery, a racing heart, trouble sleeping, or an upset stomach, basically caffeine effects that feel stronger than expected. This is a theoretical concern based on how these compounds behave, not something proven to cause big problems, and a cup or two of green tea is unlikely to be an issue for most folks. Your pill still works fine. If caffeine bothers you, chat with your pharmacist.

ESTROGENS

3 of 6

Ethinyl Estradiol, Norgestrel also falls under ESTROGENS.

Theoretically, estrogens might increase the levels and adverse effects of caffeine.
Green tea contains caffeine. Estrogen inhibits caffeine metabolism.

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's a twist that surprises a lot of people: this interaction isn't really your birth control pill (Ovral, Lo/Ovral and similar) building up. It's the other way around. The estrogen in your pill can slow down how your body clears caffeine, and green tea naturally contains caffeine. So the caffeine from your tea may hang around longer and hit a little harder than usual.

In real life that might mean more jitteriness, a racing heart, trouble sleeping, or an upset stomach, especially if you also drink coffee or energy drinks. A cup or two of green tea is usually no big deal. Concentrated green tea extract pills are where you'd notice it more. If caffeine bothers you, mention it to your pharmacist or doctor before changing anything.

P-GLYCOPROTEIN SUBSTRATES

4 of 6

Ethinyl Estradiol, Norgestrel also falls under P-GLYCOPROTEIN SUBSTRATES.

Green tea might increase the levels and adverse effects of P-glycoprotein (P-gp) substrates.
In vitro research and case reports suggest that green tea inhibits drug efflux by P-gp, potentially increasing serum levels of P-gp substrates. Case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking green tea and certain P-gp substrates.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for your birth control pill (Ovral and similar combos of ethinyl estradiol and norgestrel). Green tea may gently block a cellular pump called P-glycoprotein, which helps move some drugs out of your body. If that pump slows down, a bit more of the hormone could linger in your bloodstream. In theory, that might mean slightly more of the usual estrogen-type side effects, like nausea, breast tenderness, bloating, or headaches.

I want to be honest about the evidence here: it's weak, based mostly on lab studies and a handful of scattered case reports. Drinking a cup or two of green tea is very unlikely to cause trouble. Concentrated green tea extract pills are the bigger question mark. If you take those, just give your pharmacist or doctor a heads-up first.

CONTRACEPTIVE DRUGS

5 of 6

Ethinyl Estradiol, Norgestrel also falls under CONTRACEPTIVE DRUGS.

Theoretically, concomitant use might increase the effects and adverse effects of caffeine found in green tea.
Green tea contains caffeine. Oral contraceptives can decrease caffeine clearance by 40% to 65%.
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's something a little different about this pairing. The combination pill you're taking (Ovral and its generics) doesn't get blocked or boosted by green tea. Instead, it works the other way around: birth control pills slow down how fast your body clears caffeine, and green tea naturally contains caffeine. So the caffeine from your tea can hang around longer and hit harder than usual.

In real life, that might mean feeling more jittery, a racing or pounding heart, trouble sleeping, or an upset stomach, especially if you drink several cups a day or take concentrated green tea extracts. Your contraceptive itself still works fine. This is mostly about caffeine sensitivity. If the buzz feels like too much, cut back a bit or chat with your pharmacist.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

6 of 6

Ethinyl Estradiol, Norgestrel also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Green tea is unlikely to produce clinically significant changes in the levels and clinical effects of CYP3A4 substrates.
In vitro and in vivo research suggests that green tea can inhibit intestinal CYP3A and induce hepatic CYP3A4 enzymes. However, this effect is unlikely to be clinically significant, as green tea does not appear to affect CYP3A4 activity in humans.
Severity Minor Likelihood Unlikely Evidence D

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Good news here: the birth control combo in pills like Ovral or Lo/Ovral (ethinyl estradiol plus norgestrel) is handled by an enzyme called CYP3A4, and green tea can tug on that enzyme a little in lab dishes. But in real people, drinking green tea hasn't been shown to meaningfully change how this enzyme works. So your daily cup or two isn't expected to weaken your pill or cause extra estrogen side effects like nausea, breast tenderness, or breakthrough bleeding.

This is based on test-tube and animal research, not human problems, so I'd put it in the 'don't lose sleep over it' category. If you're taking concentrated green tea extract pills rather than the beverage, it's still worth a quick chat with your pharmacist just to be safe.

About Green Tea

What is Green Tea?

Green Tea
  • Antioxidant support
  • Weight loss
  • Heart health
  • Mental alertness
  • Cancer risk reduction

Green tea is a popular beverage rich in antioxidants called catechins, and drinking it in normal amounts is considered safe for most people. Concentrated green tea extracts are a different story—they may help modestly with weight or heart health for some, but high doses have been linked to rare liver problems. Talk to your pharmacist or doctor before taking green tea supplements, especially if you have liver concerns or are pregnant.
