Green Tea and Loncastuximab Tesirine-Lpyl

Here is the interaction we found between these two, with the evidence behind it.

Moderate

P-GLYCOPROTEIN SUBSTRATES

Why this applies to Loncastuximab Tesirine-Lpyl

Loncastuximab Tesirine-Lpyl falls under P-GLYCOPROTEIN SUBSTRATES — the group Green Tea interacts with.

Green tea might increase the levels and adverse effects of P-glycoprotein (P-gp) substrates.
In vitro research and case reports suggest that green tea inhibits drug efflux by P-gp, potentially increasing serum levels of P-gp substrates. Case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking green tea and certain P-gp substrates.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the idea behind this one. Green tea, especially in concentrated extract form, may block a cellular pump called P-glycoprotein that helps move certain drugs out of your system. If that pump slows down, levels of a P-gp drug can creep up, which could mean stronger effects and more side effects. With Zynlonta (loncastuximab tesirine), that might look like worse low blood counts, swelling, fatigue, rash, or liver irritation.

I want to be honest about the strength of this, though. It rests on lab studies and a few scattered case reports, so it's far from proven. Everyday cups of green tea are unlikely to cause trouble; high-dose extracts are the bigger question. Since Zynlonta is a serious cancer therapy, please run any green tea supplement past your oncology team or pharmacist first.

From our pharmacist’s overview on the Loncastuximab Tesirine-Lpyl page

“Loncastuximab tesirine (Zynlonta) is an infusion treatment for lymphoma that has come back or stopped responding, so I treat anything that can shift its levels as a real safety question. All 37 ingredients on file interact through the same doorway, a transporter called P-glycoprotein that moves drugs across cell membranes.”

About Green Tea

What is Green Tea?

Green Tea
  • Antioxidant support
  • Weight loss
  • Heart health
  • Mental alertness
  • Cancer risk reduction

Green tea is a popular beverage rich in antioxidants called catechins, and drinking it in normal amounts is considered safe for most people. Concentrated green tea extracts are a different story—they may help modestly with weight or heart health for some, but high doses have been linked to rare liver problems. Talk to your pharmacist or doctor before taking green tea supplements, especially if you have liver concerns or are pregnant.
