CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Gilteritinib Fumarate
Gilteritinib Fumarate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Gymnema interacts with.
One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.

Here's the short version: your leukemia medicine, gilteritinib (Xospata), is cleared from your body in part by a liver enzyme called CYP3A4. Gymnema has been looked at to see if it slows that enzyme down. In theory, if it did, gilteritinib could build up a bit and you might feel more of its side effects, things like fatigue, muscle aches, swelling, diarrhea, or changes that show up on heart and lab monitoring.
The reassuring part is that this is very weak, mostly test-tube and animal data, and the better human-tissue studies suggest gymnema does not meaningfully affect CYP3A4. So a real-world problem is unlikely. Still, because gilteritinib is a serious cancer drug, please run any gymnema use past your oncology team before starting it.

Gilteritinib, sold as Xospata tablets, treats adults with acute myeloid leukemia (AML) that has come back (relapsed) or has not responded to earlier treatment (refractory).
It is only used when a test approved by the FDA shows the leukemia has a FLT3 mutation (a change in the FLT3 gene) in the blood or bone marrow.
