Why this applies to Larotrectinib

Larotrectinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Gymnema interacts with.

Theoretically, gymnema might increase levels of drugs metabolized by CYP3A4.
One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.

Here's the gist for Vitrakvi (larotrectinib): your body clears this cancer drug mainly through a liver enzyme called CYP3A4. The thinking is that gymnema might slow that enzyme down a little, which in theory could let larotrectinib build up and bump up side effects like dizziness, fatigue, nausea, or changes in your liver lab values.

I want to be straight with you, though, this is a very weak, mostly theoretical concern. It comes from a single rat-tissue study, while human cell studies and live animal tests showed no real effect on this enzyme. Still, because larotrectinib is a serious cancer treatment, please don't add gymnema without first running it by your oncology team or pharmacist.

What Is Larotrectinib Used For?

Brand name: Vitrakvi

Vitrakvi is used to treat adult and pediatric patients with solid tumors that have a neurotrophic receptor tyrosine kinase (NTRK) gene fusion — a specific genetic alteration — without a known acquired resistance mutation. It is used when the tumor has spread (is metastatic) or when surgery would likely cause severe harm, and when there are no satisfactory alternative treatments or the cancer has continued to grow after prior treatment.

Patients are selected for Vitrakvi based on an FDA-approved test that confirms the NTRK gene fusion in tumor tissue. This approval was granted under the FDA's accelerated approval pathway, based on overall response rate and duration of response; continued approval may depend on results from confirmatory trials.
