Levobupivacaine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Gymnema interacts with.

Theoretically, gymnema might increase levels of drugs metabolized by CYP1A2.
Animal and in vitro research shows that gymnema can inhibit the CYP1A2 enzyme. In one animal study, oral administration of gymnema for 7 days increased the plasma concentrations of phenacetin, a CYP1A2 substrate, by about 1.4-fold and reduced the clearance of phenacetin by about 29%.

Here's the gist for levobupivacaine (Chirocaine), which is a local anesthetic your doctor injects to numb an area or block a nerve. Your liver partly clears it using an enzyme called CYP1A2. Gymnema may slow that enzyme down a bit, so in theory the drug could hang around a little longer and build up. That could mean a stronger or longer numbing effect, and in higher amounts, side effects like dizziness, ringing in the ears, a numb mouth, or, rarely, heart rhythm or seizure concerns.

The good news: this is only theoretical, based on lab and animal data, not real human cases. Also, levobupivacaine is usually given in a controlled setting by a professional. Still, mention any gymnema use to your anesthesia provider or pharmacist beforehand so they can keep an eye on it.

Theoretically, gymnema might increase levels of drugs metabolized by CYP3A4.
One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.

Levobupivacaine (Chirocaine) is a local anesthetic, usually given by an anesthesia provider during surgery or for pain control. Part of how your body clears it relies on a liver enzyme called CYP3A4. The theory here is that gymnema might slow that enzyme down a little, which could let levobupivacaine linger longer and, in theory, raise the odds of side effects like numbness lasting longer, dizziness, or heart rhythm changes.

Honestly, the evidence behind this is very thin. It comes from rat liver studies, while human and other animal data suggest gymnema does not meaningfully affect CYP3A4. Since this drug is given in a controlled medical setting, just let your care team know about any gymnema you take so they have the full picture.
