Why this applies to Mavacamten

Mavacamten falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Gymnema interacts with.

Theoretically, gymnema might increase levels of drugs metabolized by CYP3A4.
One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.

Here's the short version: there's a theoretical concern that Gymnema could slow down the liver enzyme (CYP3A4) that helps clear Mavacamten (Camzyos) from your body. If that happened, Mavacamten levels could creep up, and since this drug works by easing how hard your heart muscle squeezes, too much can drop your heart's pumping strength too far. That can mean shortness of breath, dizziness, swelling, or feeling worn out.

The good news is that the evidence here is very weak. It comes from a rat lab study, while human liver studies and other animal work showed no real effect. Still, Mavacamten is a drug that's carefully dose-adjusted, so even small shifts matter. Please run Gymnema by your cardiologist or pharmacist before adding it.

What Is Mavacamten Used For?

Brand name: Camzyos

Camzyos (mavacamten) is approved for adults with symptomatic obstructive hypertrophic cardiomyopathy (HCM) — a condition where the heart muscle grows too thick and blocks blood flow out of the heart. Specifically, it is used in people whose symptoms fall into New York Heart Association (NYHA) class II or III, meaning they have noticeable limitations on physical activity.

The goal of treatment is to improve how much the patient can do physically and to reduce the symptoms that HCM causes, such as shortness of breath, chest pain, and fatigue.
