Warfarin falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Gymnema interacts with.
Animal and in vitro research shows that gymnema can inhibit the CYP1A2 enzyme. In one animal study, oral administration of gymnema for 7 days increased the plasma concentrations of phenacetin, a CYP1A2 substrate, by about 1.4-fold and reduced the clearance of phenacetin by about 29%.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with warfarin and gymnema. Warfarin is a blood thinner that your liver clears partly through an enzyme called CYP1A2. Lab and animal studies hint that gymnema can slow this enzyme down a bit. If that happened in people, warfarin could build up slightly, which in theory might nudge your blood toward thinning too much. With warfarin, even small shifts matter, since too much can mean bruising, nosebleeds, or more serious bleeding.
That said, the evidence here is weak and mostly theoretical, not from real human warfarin studies. I'm not telling you to avoid it, just don't start gymnema on your own. Check with your pharmacist or doctor first, and keep up with your regular INR checks so any change gets caught early.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Animal research shows that gymnema can induce the CYP2C9 enzyme. In one animal study, gymnema caused a 2.4-fold increase in the clearance of tolbutamide, a CYP2C9 substrate, in rats. In vitro research also shows that gymnema can inhibit CYP2C9.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing with warfarin: it's a 'blood thinner' that your liver clears using an enzyme called CYP2C9. Gymnema may nudge that enzyme in either direction. In lab and animal studies it has both sped the enzyme up and slowed it down, so we honestly can't predict which way it would tip your warfarin. If it speeds clearance, your warfarin could weaken and clots become more likely. If it slows clearance, levels could climb and raise your bleeding risk (easy bruising, nosebleeds, blood in urine or stool).
Warfarin is a narrow-therapeutic-index drug, meaning even small swings matter and show up on your INR. This is only theoretical right now, so don't panic, but please loop in your pharmacist or doctor before adding gymnema. Extra INR checks may be wise.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.

Here's the honest picture with warfarin and gymnema. The theory on file is that gymnema might slow down a liver enzyme called CYP3A4, which could let drugs cleared by that pathway build up a little. On paper, that would mean slightly more warfarin effect and a higher chance of bleeding signs like easy bruising, nosebleeds, bleeding gums, or dark stools.
But the evidence here is very weak. One test-tube study using rat liver hinted at this, while human-cell and animal studies showed no real effect. So this is mostly theoretical. The bigger, more practical concern is that gymnema may lower blood sugar and could nudge your INR. Because warfarin is so dose-sensitive, please loop in your pharmacist or doctor before adding it, and keep up your regular INR checks.

What Is Warfarin Used For?

Warfarin sodium tablets and Jantoven tablets are both used to prevent and treat venous thrombosis (blood clots in the veins) and pulmonary embolism (a clot in the lungs). They are also used to prevent clot-related complications in people with atrial fibrillation (an irregular heart rhythm) or who have had a heart valve replaced.
In people who have had a heart attack, warfarin is used to reduce the risk of death, another heart attack, stroke, or other clotting events. It does not dissolve an existing clot, but it helps prevent the clot from growing and stops new dangerous clots from forming.
