Most clinical evidence suggests that taking Hibiscus sabdariffa reduces systolic and diastolic blood pressure. In animal research, Hibiscus sabdariffa increased the hypotensive effects of single doses of losartan and amlodipine.

Ziac is a two-in-one blood pressure pill: bisoprolol (a beta-blocker) plus hydrochlorothiazide (a water pill). Both already work to bring your numbers down. Hibiscus, especially as a regular tea or concentrated supplement, has a mild blood-pressure-lowering effect of its own. So the concern here is simple addition: putting them together could nudge your pressure a little lower than intended.

If that happens, you might feel lightheaded, dizzy, tired, or shaky when you stand up too fast. To be clear, this interaction is theoretical and based mostly on how things work in the body and some animal studies, not strong human proof. An occasional cup of hibiscus tea is unlikely to cause trouble. Before using it daily or as a supplement, just check with your pharmacist or doctor.

Bisoprolol, Hydrochlorothiazide also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.

Theoretically, Hibiscus sabdariffa extract might reduce the metabolism of CYP2D6 substrates.
In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP2D6. This interaction has not been reported in humans.

Here's the gist with your Ziac, which combines bisoprolol (a beta blocker) and hydrochlorothiazide (a water pill). The bisoprolol part is partly cleared by a liver enzyme called CYP2D6. Lab studies in test tubes suggest hibiscus might slow that enzyme down a little, which could in theory let a bit more bisoprolol linger in your system. If that happened, you might notice things like a slower heart rate, more tiredness, lightheadedness, or your blood pressure dropping a touch lower than usual.

That said, this is purely theoretical. It has never actually been reported in people, and hibiscus itself can already nudge blood pressure down. A cup of hibiscus tea is unlikely to cause trouble, but if you're using concentrated supplements, give your pharmacist or doctor a quick heads-up first.
