Hibiscus Sabdariffa and Clomipramine

These two interact in 5 distinct ways. Each one is explained separately below.

Why Are There 5 Interactions?

Hibiscus Sabdariffa interacts with whole categories of medication. Clomipramine falls into 5 of those categories, so it can interact with Hibiscus Sabdariffa in 5 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Minor).

Minor

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

1 of 5

Why this applies to Clomipramine

Clomipramine falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Hibiscus Sabdariffa interacts with.

Theoretically, Hibiscus sabdariffa extract might reduce the metabolism of CYP2C19 substrates.
In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP2C19. This interaction has not been reported in humans.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: your body uses a liver enzyme called CYP2C19 to help break down clomipramine. In lab dish studies, hibiscus extract appears to slow that enzyme down a little. If that happened in real life, clomipramine could clear out more slowly and build up, which might nudge up its usual side effects like dry mouth, constipation, drowsiness, dizziness, blurry vision, or a faster heartbeat.

The reassuring part: this is only theoretical, seen in test tubes and never actually reported in people. A cup or two of tart hibiscus tea is unlikely to be a problem. If you're using a concentrated hibiscus supplement, it's worth a quick chat with your pharmacist or doctor before starting. Please don't change your clomipramine on your own.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

2 of 5

Clomipramine also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, Hibiscus sabdariffa extract might reduce the metabolism of CYP1A2 substrates.

Here's the gist with hibiscus and your clomipramine. One of the liver enzymes that helps clear this medication is called CYP1A2. In lab dish studies, concentrated hibiscus extract appears to slow that enzyme down a little. If that happened in the body, your clomipramine could clear a bit more slowly and build up, which in theory might mean stronger effects or more of the usual tricyclic side effects like drowsiness, dry mouth, constipation, or feeling lightheaded when you stand.

The reassuring part: this is only a theory from test-tube work and has never actually been reported in people. A cup or two of hibiscus tea is very unlikely to matter. Concentrated hibiscus supplements deserve a quick check, so just mention them to your pharmacist or doctor.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

3 of 5

Clomipramine also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.

Theoretically, Hibiscus sabdariffa extract might reduce the metabolism of CYP2D6 substrates.
In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP2D6. This interaction has not been reported in humans.

Here's the simple version: your body uses a liver enzyme called CYP2D6 to help break down clomipramine. In test-tube studies, concentrated hibiscus extract slowed that same enzyme down a bit. If that happened in real life, clomipramine could clear a little slower and build up slightly, which in theory might bump up its usual side effects, things like dry mouth, constipation, drowsiness, dizziness, or a faster heartbeat.

I want to keep this in perspective. This is only a theory from lab work and hasn't been seen in actual people, so the concern is small. A cup or two of hibiscus tea is unlikely to matter much, while strong supplement doses deserve more caution. Since clomipramine is a sensitive medicine, just run it by your pharmacist or doctor first.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

4 of 5

Clomipramine also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, Hibiscus sabdariffa extract might reduce the metabolism of CYP2C9 substrates.
In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP2C9. This interaction has not been reported in humans.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with hibiscus and your clomipramine. Lab studies (done in test tubes, not people) suggest hibiscus extract may slow down an enzyme called CYP2C9, which is one of the pathways that helps clear this medication. If that enzyme works a little slower, clomipramine could build up slightly, which in theory might nudge up its usual side effects like dry mouth, constipation, drowsiness, dizziness on standing, or a faster heartbeat.

That said, this is very much a theoretical concern. It has never actually been reported in real patients, and clomipramine is mostly handled by other enzymes too. A cup of hibiscus tea is unlikely to cause trouble, but if you're using concentrated hibiscus supplements, just give your pharmacist or doctor a heads-up first.

Minor

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

5 of 5

Clomipramine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, Hibiscus sabdariffa extract might reduce the metabolism of CYP3A4 substrates.
In vitro research shows that Hibiscus sabdariffa calyx extract inhibits CYP3A4. This interaction has not been reported in humans.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for clomipramine (Anafranil): your body partly uses an enzyme called CYP3A4 to break this medicine down. Lab studies in a dish suggest hibiscus might slow that enzyme a bit. In theory, that could let a little more clomipramine build up, which might nudge up its usual side effects, things like dry mouth, constipation, drowsiness, blurry vision, or a faster heartbeat.

I want to be honest about the strength here: this is only a test-tube finding and has never been seen in real people. Occasional cups of tart hibiscus tea are very unlikely to cause trouble. Concentrated hibiscus supplements are the bigger question mark. If you take clomipramine daily, it's worth a quick chat with your pharmacist or doctor before adding hibiscus regularly.

From our pharmacist’s overview on the Clomipramine page

“A tricyclic antidepressant used mostly for OCD, clomipramine (Anafranil) is unforgiving of shifts in its blood level, which is why its 236 supplement interactions deserve genuine wariness. Most of the list involves metabolism: herbs that speed up or slow down the liver enzymes handling clomipramine can drag its level too low or push it…”

About Hibiscus Sabdariffa

What is Hibiscus Sabdariffa?

Hibiscus Sabdariffa
  • High blood pressure
  • Heart and cholesterol health
  • Digestive upset
  • Antioxidant support
  • Fever and general wellness

Hibiscus sabdariffa is a tart, cranberry-flavored plant most often consumed as a tea, and its best-studied use is for mildly lowering blood pressure. Evidence for other uses is limited, and while hibiscus tea is generally well tolerated in food amounts, concentrated supplements should be used cautiously and avoided in pregnancy. Talk to your pharmacist or doctor before using it for a medical condition.
