Most clinical evidence suggests that taking Hibiscus sabdariffa reduces systolic and diastolic blood pressure. In animal research, Hibiscus sabdariffa increased the hypotensive effects of single doses of losartan and amlodipine.

Here's the simple version: both hibiscus and losartan (Cozaar) work to lower your blood pressure. So when you combine them, those effects can stack on top of each other and push your numbers down a little more than expected. That could leave you feeling lightheaded, dizzy, tired, or wobbly when you stand up quickly.

I want to be honest about how strong this concern is, though. The worry comes mostly from animal studies and pharmacology theory, not solid human trials, so it's fairly mild. A cup of hibiscus tea now and then is unlikely to cause trouble. Concentrated hibiscus supplements taken daily are where I'd pay closer attention. If you're using either regularly, check your blood pressure at home and chat with your pharmacist or doctor.

Theoretically, Hibiscus sabdariffa might increase the levels and clinical effects of losartan.
Animal research in rats with laboratory-induced hypertension shows that providing Hibiscus sabdariffa for 14-17 days prior to a single administration with losartan modestly increases losartan concentrations and increases hypotensive effects when compared with a single administration of losartan alone. It is not clear if Hibiscus sabdariffa alters the concentration or effects of losartan when taken continuously. Additionally, this interaction has not been shown in humans.

Here's the short version on mixing hibiscus tea with your losartan (Cozaar). Both lower blood pressure, and there's a thought that hibiscus might also nudge losartan levels up a bit, so the combined effect could be a little stronger than either alone. If that happened, you might notice signs of blood pressure dipping too low: lightheadedness, dizziness when you stand up, or feeling unusually tired.

That said, I want to keep this in perspective. The only evidence we have comes from a rat study, not people, and it hasn't been shown to matter in humans. A cup of hibiscus tea now and then is unlikely to cause trouble. Concentrated hibiscus supplements are where I'd be more cautious. If you use hibiscus regularly, just mention it to your pharmacist or doctor so they can keep an eye on your readings.

Here's something worth knowing about losartan. Your body uses an enzyme called CYP2C9 to turn losartan into its more active form, so losartan is a bit like a prodrug. Lab studies (done in test tubes, not people) suggest hibiscus may slow down that same enzyme. In theory, that could mean slightly less of the active form, which might make losartan a touch less effective at lowering your blood pressure.

I want to be honest with you though, this is purely theoretical and has never been seen in actual patients. A cup or two of hibiscus tea is unlikely to cause problems, and hibiscus itself tends to nudge blood pressure down. If you take concentrated hibiscus supplements, just mention it to your pharmacist or doctor and keep an eye on your home blood pressure readings.

Here's something interesting about losartan: it actually relies on your liver enzymes (mostly CYP2C9, with some CYP3A4 involvement) to turn it into its more active form. So losartan behaves a bit like a prodrug. If hibiscus slows down CYP3A4, the concern in theory is that you'd make slightly less of that active piece, which could blunt losartan's blood-pressure-lowering punch a little.

But take a deep breath here. This is purely a lab-dish finding, and it has never actually been seen in people. On top of that, hibiscus itself gently lowers blood pressure, so any real-world effect is likely small. A cup of tart hibiscus tea is very unlikely to cause trouble. If you're using concentrated capsules or checking your blood pressure regularly, just give your pharmacist or doctor a heads-up.

What Is Losartan Used For?

Brand name: Cozaar

Losartan tablets — available as branded Cozaar and several generic products — are used to treat high blood pressure (hypertension) in adults and children 6 years and older. Lowering blood pressure reduces the risk of strokes and heart attacks. Losartan tablets are also used to reduce the risk of stroke in people with high blood pressure who also have left ventricular hypertrophy (an enlarged left side of the heart). Note: evidence suggests this stroke-risk benefit may not apply to Black patients.

Losartan tablets are additionally approved to protect the kidneys in people with type 2 diabetes, high blood pressure, and signs of kidney damage (diabetic nephropathy). Arbli, the oral suspension form, shares these same approved uses.
