Imipramine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Hops interacts with.
In vitro research suggests that flavonoid constituents of hops inhibit CYP1A2 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, does not affect levels of caffeine, a CYP1A2 probe substrate.

Here's the short version with hops and imipramine (Tofranil): there's very little to worry about. The concern comes from lab dish studies where certain hops flavonoids slowed down an enzyme called CYP1A2, which is one of the pathways your body uses to clear imipramine. In theory, blocking that pathway could let imipramine build up and bring on more of its usual side effects, like daytime drowsiness, dry mouth, constipation, or feeling lightheaded when you stand up.
But here's the reassuring part: when researchers actually tested a standardized hops extract in real people, it did not change how the body processes a CYP1A2 marker. So this is a theoretical, test-tube finding, not something seen in humans. One real overlap worth mentioning is that both hops and imipramine can make you sleepy. If you're considering hops, just run it by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for imipramine (Tofranil) and hops. Your body uses an enzyme called CYP3A4 as one way to break down imipramine. Lab and animal studies hint that hops might speed that enzyme up a bit, which in theory could clear imipramine a little faster and slightly soften its effect. Imipramine is not a prodrug, so faster clearance means less drug around, not more.
Honestly, this is very weak, mostly test-tube evidence. In a human study, a hops extract changed a similar drug's levels by under 8 percent, which isn't enough to matter. So I wouldn't lose sleep over it. Still, since imipramine can cause drowsiness and hops can too, run the combo past your pharmacist or doctor before starting.

Hops are most often used for sleep and mild anxiety, frequently combined with valerian, but the human evidence is limited and not very strong. What Is Imipramine Used For?
Brand names: Tofranil, Tofranil PM
Imipramine is used to relieve symptoms of depression. Both Imipramine Hydrochloride tablets and Imipramine Pamoate capsules are approved for this use. Endogenous depression — the kind that seems to arise from within rather than a clear outside cause — tends to respond especially well. It may take one to three weeks before you notice the full benefit.
Imipramine Hydrochloride tablets are also approved as a short-term, add-on treatment to help reduce nighttime bedwetting (nocturnal enuresis) in children aged 6 and older, after a doctor has ruled out physical causes.
