Why this applies to Tadalafil

Tadalafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hops interacts with.

Theoretically, hops extract might alter metabolism of CYP3A4 substrates; however, this effect may not be clinically significant.
Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.

Here's the gist for tadalafil (Cialis): your liver uses an enzyme called CYP3A4 to clear it out of your system. Hops might nudge that enzyme to work a little faster, which in theory could lower tadalafil levels and make it slightly less effective for things like erectile dysfunction or your BPH urinary symptoms.

But I want to keep this in perspective. This concern comes mostly from lab and animal studies, and one human study found only about an 8% change with a different drug, which is too small to really notice. Tadalafil is not a prodrug, so it doesn't flip the other way. If you take hops for sleep and feel your tadalafil isn't working as well, just mention it to your pharmacist or doctor.

What Is Tadalafil Used For?

Brand names: Cialis, Tadliq

Tadalafil tablets (including Cialis and generic tadalafil) are used to treat erectile dysfunction (ED), the symptoms of benign prostatic hyperplasia (BPH — an enlarged prostate), or both conditions together. When used with the drug finasteride to start BPH treatment, this combination is recommended for up to 26 weeks.

Adcirca, Alyq (tablets), and Tadliq (oral suspension) are indicated specifically for pulmonary arterial hypertension (PAH — high blood pressure in the lungs), to improve the ability to exercise.
