Why this applies to Tadalafil

Tadalafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Gymnema interacts with.

Theoretically, gymnema might increase levels of drugs metabolized by CYP3A4.
One in vitro study using rat liver microsomes shows that gymnema can modestly inhibit the CYP3A4 enzyme. However, other in vitro research using human liver microsomes shows that gymnema does not affect CYP3A4 activity. Animal research also shows that gymnema does not alter the function of CYP3A4. In one study in rats, oral administration of gymnema for 7 days did not alter the clearance of amlodipine, a CYP3A4 substrate.

Here's the gist with Gymnema and tadalafil (Cialis). Your body clears tadalafil mainly through a liver enzyme called CYP3A4. The theory is that gymnema could slow that enzyme down a little, which in turn could let tadalafil linger a bit longer and nudge up its effects, things like headache, flushing, stuffy nose, back pain, or lightheadedness from a drop in blood pressure.

That said, I wouldn't lose sleep over this one. The only hint of an effect came from a test-tube study using rat liver, while human-tissue and live-animal studies showed no real change. So this is very much a theoretical, low-likelihood concern. Still, since gymnema may also lower blood sugar, it's worth a quick word with your pharmacist or doctor before combining them.

About Tadalafil

What Is Tadalafil Used For?

Brand names: Cialis, Tadliq

Tadalafil tablets (including Cialis and generic tadalafil) are used to treat erectile dysfunction (ED), the symptoms of benign prostatic hyperplasia (BPH — an enlarged prostate), or both conditions together. When used with the drug finasteride to start BPH treatment, this combination is recommended for up to 26 weeks.

Adcirca, Alyq (tablets), and Tadliq (oral suspension) are indicated specifically for pulmonary arterial hypertension (PAH — high blood pressure in the lungs), to improve the ability to exercise.
