Flupenthixol falls under SEROTONERGIC DRUGS — the group Iboga interacts with.

Theoretically, taking iboga with serotonergic drugs might increase levels of serotonin, resulting in additive effects with serotonergic drugs.
Ibogaine, a constituent in iboga, has serotonergic effects. Theoretically, combining serotonergic drugs with iboga might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. Monitor patients for signs of serotonin syndrome and other serotonergic side effects if using iboga with serotonergic drugs.

Here's the gist with Flupenthixol, an antipsychotic that's also used in low doses for depression. Iboga (through its main alkaloid, ibogaine) nudges up serotonin activity in the brain. Flupenthixol can mildly raise serotonin too, so the worry is that stacking the two could push serotonin levels higher than intended.

If that happened, you might see signs of serotonin syndrome: agitation, a racing heart, sweating, shivering, muscle twitching, or confusion. I'll be honest, this concern is theoretical, based on how each acts rather than real-world reports. The bigger issue is that ibogaine itself carries serious heart rhythm risks and is illegal here. Please don't combine these without talking to your doctor or pharmacist first.

Here's the concern with combining these two. Flupenthixol is an older antipsychotic that, like many in its class, can stretch out the heart's electrical recovery time, something doctors call QT prolongation. Iboga (through its main compound, ibogaine) has been linked in real-world case reports to that same effect. Stack the two and the risk may add up, which in rare cases can trigger a dangerous, fast heart rhythm with symptoms like fainting, a racing or pounding heartbeat, or feeling lightheaded.

This isn't from rigorous studies, just scattered case reports, so the evidence is fairly thin. Even so, I'd treat it seriously, because Iboga itself carries real cardiac risks and is illegal in the U.S. Please don't start or stop anything on your own. Talk it through with your doctor or pharmacist first.
