Abrocitinib falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ipriflavone interacts with.

Ipriflavone is thought to competitively inhibit cytochrome P450 2C9 (CYP2C9). Theoretically, concurrent use of ipriflavone and drugs metabolized by these enzymes might result in decreased drug elimination, increased serum levels, and potential toxicity. Some drugs metabolized by CYP2C9 include amitriptyline (Elavil), diazepam (Valium), estradiol (Estrace), tacrine (Cognex), verapamil (Calan), warfarin (Coumadin), zileuton (Zyflo), and others.

Here's the thinking on this pairing. Your eczema medicine, abrocitinib (Cibinqo), is partly cleared by a liver enzyme called CYP2C9. Ipriflavone is believed to slow that same enzyme down. In theory, that could mean your body clears abrocitinib a little slower, letting levels drift higher than expected. Abrocitinib is an active drug, not a prodrug, so higher levels generally mean stronger effects and a greater chance of side effects like nausea, headache, acne, or a deeper dip in blood cell counts.

One important point: this concern comes only from lab data, not from real patient studies, so it may not pan out in practice. Since abrocitinib already affects your immune system and blood counts, it's worth a quick chat with your pharmacist or doctor before combining them.

Concurrent use of ipriflavone and immunosuppressant drugs might have additive effects on inhibiting immune function. Some patients taking ipriflavone experience subclinical lymphocytopenia; avoid concurrent use. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), and other corticosteroids (glucocorticoids).

Here's the key thing with Cibinqo (abrocitinib) and ipriflavone. Abrocitinib is a JAK inhibitor, which means it already dials down your immune system on purpose to calm your eczema. One known side effect is lower blood cell counts, including white cells (lymphocytes). Ipriflavone has been shown in some people to also lower white blood cell counts. So this isn't about one drug speeding up or slowing down the other. It's that both can nudge your immune defenses and blood counts in the same direction at the same time.

Stacked together, that could mean a higher chance of low white cells and a greater risk of catching infections. This is why the guidance leans toward avoiding the combination. Please don't change either one on your own. Talk with your pharmacist or doctor first so they can check your bloodwork and steer you safely.

What Is Abrocitinib Used For?

Brand name: Cibinqo

Cibinqo (abrocitinib) is used to treat moderate-to-severe atopic dermatitis (eczema) in adults and children 12 years of age and older. It is specifically for people whose eczema has not been adequately controlled with other systemic (whole-body) treatments, including biologics, or when those treatments are not a safe option.

Cibinqo is not recommended to be used at the same time as other JAK inhibitors, biologic immunomodulators, or other immunosuppressants (medicines that reduce immune system activity).
