Effient falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ipriflavone interacts with.

Ipriflavone is thought to competitively inhibit cytochrome P450 2C9 (CYP2C9). Theoretically, concurrent use of ipriflavone and drugs metabolized by these enzymes might result in decreased drug elimination, increased serum levels, and potential toxicity. Some drugs metabolized by CYP2C9 include amitriptyline (Elavil), diazepam (Valium), estradiol (Estrace), tacrine (Cognex), verapamil (Calan), warfarin (Coumadin), zileuton (Zyflo), and others.

Here's the thing about Effient (prasugrel): it's actually a prodrug, meaning it has to be switched on by your liver enzymes before it can do its job of keeping platelets from clumping. The main enzymes that activate it are CYP3A4 and CYP2B6, not CYP2C9, which is the enzyme ipriflavone is thought to block. So this particular pathway isn't the one prasugrel really depends on.

On top of that, the concern here is purely theoretical, based only on lab and animal data, not real patients. I wouldn't expect ipriflavone to meaningfully change how well your Effient works. Still, since Effient and aspirin both raise bleeding risk, and ipriflavone's long-term safety isn't well studied, please run this combination by your pharmacist or doctor before starting it.

What Is Prasugrel Used For?

Brand name: Prasugrel

Prasugrel tablets (brand names Effient and Prasugrel) are used to lower the risk of dangerous blood clots — including clots that can block a stent — in people with acute coronary syndrome (ACS), which includes unstable angina (chest pain at rest) and heart attacks (both NSTEMI and STEMI types). It is used specifically in patients who are being treated with a procedure called percutaneous coronary intervention (PCI), in which a catheter is used to open a blocked coronary artery, often with a stent.

Effient has been shown to reduce the combined rate of cardiovascular death, nonfatal heart attack, or nonfatal stroke compared to clopidogrel — with the benefit driven mainly by fewer heart attacks.
