Ramelteon falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ipriflavone interacts with.

Ipriflavone is thought to competitively inhibit cytochrome P450 1A2 (CYP1A2). Theoretically, concurrent use of ipriflavone and drugs metabolized by these enzymes might result in decreased drug elimination, increased serum levels, and potential toxicity. Some drugs metabolized by CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), Pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), theophylline (Slo-bid, Theo-Dur, others), zileuton (Zyflo), Zolmitriptan (Zomig), and others.

Here's the connection worth knowing. Your body relies on a liver enzyme called CYP1A2 to clear ramelteon (Rozerem) out of your system. Ipriflavone is thought to slow that same enzyme down, so in theory ramelteon could hang around longer and reach higher levels than usual. Ramelteon is the active drug itself (not a prodrug), so more of it means a stronger effect.

In practice, that might show up as feeling extra drowsy, groggy, or dizzy the next morning. I want to be honest, though: this interaction is only based on lab studies, not real patients, so it's more of a theoretical concern than a proven one. Still, it's an easy thing to mention to your pharmacist or doctor before you pair these two.

Ipriflavone is thought to competitively inhibit cytochrome P450 2C9 (CYP2C9). Theoretically, concurrent use of ipriflavone and drugs metabolized by these enzymes might result in decreased drug elimination, increased serum levels, and potential toxicity. Some drugs metabolized by CYP2C9 include amitriptyline (Elavil), diazepam (Valium), estradiol (Estrace), tacrine (Cognex), verapamil (Calan), warfarin (Coumadin), zileuton (Zyflo), and others.

Here's the gist with ipriflavone and Ramelteon (Rozerem). Ipriflavone may slow down a liver enzyme (CYP2C9) that helps clear some medicines. Ramelteon is partly handled by this enzyme family, so in theory the supplement could let a bit more Ramelteon linger in your system. Since Ramelteon is active as-is (not a prodrug that needs switching on), more of it could mean a stronger effect, such as feeling extra drowsy, groggy, or dizzy, or being sleepy into the next morning.

That said, this concern comes only from lab and animal data, not from real patients, so it's pretty theoretical. Ramelteon is also mainly broken down by a different enzyme (CYP1A2). Still, it's worth a quick chat with your pharmacist or doctor before combining them.
