Safinamide Mesylate falls under MONOAMINE OXIDASE INHIBITORS (MAOIs) — the group Khat interacts with.

In one case report, a 24-year-old man developed reversible cerebral vasoconstriction syndrome (RCVS) thought to be caused by chewing khat in combination with treatment with tranylcypromine, an MAOI. Theoretically, the combination of khat and an MAOI might increase the induction of sympathetic stimulation, increasing the risk of vasoconstriction. Until more is known, avoid khat in patients taking MAOIs and for 14 days after MAOI discontinuation.

Here's the concern with this pairing. Safinamide (Xadago) is an MAO-B inhibitor that boosts dopamine to help with your Parkinson's 'off' times. Khat acts like an amphetamine, releasing extra norepinephrine and dopamine. When you stack a stimulant like khat on top of an MAOI, the body can't clear those stimulant signals normally, so they pile up. That can drive your blood pressure way up and tighten blood vessels.

This warning is based on a single case report (someone on a different MAOI developed a serious vessel-constriction event), so the evidence is thin and theoretical for safinamide specifically. Still, the potential outcome is serious, and khat is illegal here anyway. The safe move is to avoid khat entirely while on safinamide and for 14 days after stopping.
