Why this applies to Codeine Phosphate

Codeine Phosphate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group N,n-dmpea interacts with.

In vitro research shows that N,N-DMPEA strongly inhibits cytochrome P450 2D6 (CYP2D6). Theoretically, concomitant use with drugs metabolized by CYP2D6 might increase the risk for adverse effects from these drugs.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the twist with codeine: it's actually a prodrug. Your body uses an enzyme called CYP2D6 to convert codeine into morphine, which is what actually relieves your pain or quiets a cough. So CYP2D6 isn't clearing codeine out, it's switching it on.

Lab studies suggest N,N-DMPEA can block CYP2D6. If that happens in real people, less codeine would get turned into morphine, so you might get weaker pain relief or less cough control than expected. The good news is this is only test-tube evidence, not proven in humans, so I wouldn't panic. Still, since we know so little about N,N-DMPEA's safety, please run this combo past your pharmacist or doctor before mixing them.

Codeine Sulfate tablets are used for the management of mild to moderate pain in adults and adolescents when other treatments have not worked well enough or are not tolerated. It is an opioid medication, so it is only appropriate when non-opioid options are inadequate.

Because of its serious risks — including addiction, misuse, and overdose — codeine is reserved for patients who truly need an opioid. Prescribers are expected to use the lowest effective dose for the shortest time possible.
