Why this applies to Doxepin

Doxepin falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group N,n-dmpea interacts with.

In vitro research shows that N,N-DMPEA strongly inhibits cytochrome P450 2D6 (CYP2D6). Theoretically, concomitant use with drugs metabolized by CYP2D6 might increase the risk for adverse effects from these drugs.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the gist for doxepin: your liver leans on an enzyme called CYP2D6 to help break it down and clear it out. In lab (test tube) studies, N,N-DMPEA appears to block that very enzyme. If that blocking also happens in real people, your body could clear doxepin more slowly, letting levels creep up. Doxepin isn't a prodrug, so more drug hanging around means stronger effects, not weaker ones.

What might that feel like? More daytime grogginess, dry mouth, constipation, dizziness, or a faster heartbeat. Keep in mind this is theoretical and based only on lab data, so we don't truly know how it plays out in humans. Since N,N-DMPEA is poorly studied anyway, please run this combo by your pharmacist or doctor before mixing them.
