Dextromethorphan, Guaifenesin, Psuedoephedrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group N,n-dmpea interacts with.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the piece that matters in your cough and cold combo: dextromethorphan. Your body clears it using an enzyme called CYP2D6. Lab studies suggest N,N-DMPEA can block that enzyme, which would let dextromethorphan build up instead of clearing out normally. That could mean more of its side effects, things like drowsiness, dizziness, confusion, or feeling restless or jittery.
I'll be honest, this is mostly theory right now. The evidence comes from test-tube work, not real people, and we know very little about N,N-DMPEA in general. The pseudoephedrine in your product is already a stimulant, so stacking it with another stimulant-type supplement is worth a quick check. Please run this past your pharmacist or doctor before combining them.
