This combination product contains acetaminophen, which is one of the most common causes of liver strain when taken in high doses or over a long time. Padang Cassia is a cassia cinnamon, and it naturally carries a compound called coumarin. In large supplement amounts, coumarin has been linked to liver irritation in some sensitive people. So the worry here is simple: you'd potentially have two things nudging the liver at once.
To be clear, this is a theoretical concern based on how these substances behave, not a proven, common problem. Sprinkling cinnamon on your food is not the issue. Concentrated cinnamon capsules taken daily, plus regular acetaminophen, is where I'd pause. Please check with your pharmacist or doctor before combining them, especially if you have any liver problems or drink alcohol.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Here's the wrinkle with this combo pill. Codeine is a prodrug, meaning your body has to convert it into morphine before you feel pain relief. That conversion runs mostly through a different enzyme (CYP2D6), not the CYP3A4 pathway that lab studies suggest Padang cassia may slow down. So the cinnamon effect here is mostly theoretical, and it's based only on test-tube work, not on real people.
If anything, blocking CYP3A4 could shift a bit more codeine toward the morphine route, which might mean slightly more drowsiness, constipation, or nausea. The acetaminophen, caffeine, and salicylamide pieces aren't likely to change much. Sprinkling cinnamon on food is fine. If you take concentrated cinnamon capsules, just check with your pharmacist first.
