These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Padang Cassia interacts with whole categories of medication. Itraconazole falls into 2 of those categories, so it can interact with Padang Cassia in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
HEPATOTOXIC DRUGS
1 of 2Why this applies to Itraconazole
Itraconazole falls under HEPATOTOXIC DRUGS — the group Padang Cassia interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to know about itraconazole: it's one of those antifungals that can be tough on the liver all on its own. That matters here because Padang cassia (a cassia-type cinnamon) naturally contains coumarin, and in large amounts coumarin has been linked to liver stress. So you'd essentially be asking your liver to handle two potential irritants at the same time.
To be fair, this concern is theoretical and based mostly on the chemistry plus animal data, not solid human studies. A sprinkle of cinnamon on your oatmeal is not the worry. Concentrated cinnamon capsules taken daily are where I'd pause. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and check with your pharmacist or doctor before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Itraconazole also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for itraconazole (Sporanox, Tolsura). Your body relies on a liver enzyme called CYP3A4 to help clear this antifungal. Lab studies suggest a concentrated extract of Padang cassia may slow that enzyme down a bit. In theory, that could let more itraconazole linger in your system, which might nudge up side effects like nausea, stomach upset, headache, or, less commonly, liver strain.
I want to be honest about the evidence though. This is based only on test-tube work, not real people, so it's quite speculative. Sprinkling cinnamon on your oatmeal won't do this. Concentrated cinnamon capsules are the real question, partly because cassia also contains coumarin that can stress the liver. Since itraconazole can be hard on the liver too, please check with your pharmacist before adding a cinnamon supplement.

From our pharmacist’s overview on the Itraconazole page
“Serious fungal infections are what itraconazole (Sporanox, Tolsura) is for, and the drug travels through CYP3A4, the liver enzyme that also handles a large share of supplements. That overlap is why this list reaches 182 ingredients, and it earns genuine care; start with the six rated major.”

About Padang Cassia
What is Padang Cassia?
- Adding flavor to food
- Digestive upset
- Blood sugar support
- Antimicrobial purposes
- Antioxidant support
Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.
About Itraconazole
What Is Itraconazole Used For?
Brand names: Sporanox, Sporanox Injection, Tolsura
Itraconazole capsules (including Sporanox capsules and generic itraconazole capsules) treat serious fungal infections — blastomycosis, histoplasmosis, and aspergillosis — in both people with normal immune systems and those who are immunocompromised. Capsule forms are also approved for nail fungus (onychomycosis) caused by dermatophytes in non-immunocompromised patients. Tolsura (a gelatin-coated capsule formulation) treats blastomycosis, histoplasmosis, and aspergillosis in adults, but is not approved for nail fungus and is not interchangeable with other itraconazole products.
Itraconazole oral solution is specifically approved for oropharyngeal candidiasis (thrush — a fungal infection of the mouth) and esophageal candidiasis (a fungal infection of the food pipe). Only the oral solution has been shown to work for these mouth and throat infections; capsules cannot be substituted for this use.
