Ivosidenib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Padang Cassia interacts with.

In vitro research shows that a methanol extract of Padang cassia inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, taking Padang cassia with CYP3A4 substrates might increase the effects and side effects of these substrates.

Your body relies on a liver enzyme called CYP3A4 to clear ivosidenib (Tibsovo) out of your system. In test-tube studies, a concentrated extract of Padang cassia cinnamon slowed that enzyme down. If the same thing happened in people, ivosidenib could build up a bit higher than expected, which might mean more of its side effects, such as nausea, fatigue, diarrhea, or changes to your heart rhythm (a longer QT interval). Ivosidenib is the active drug itself, not a prodrug, so more of it in your blood simply means stronger effects.

That said, this is only laboratory evidence, not something proven in patients. Sprinkling cinnamon on your food is very unlikely to matter. Concentrated cinnamon capsules are the real question, so check with your oncology pharmacist before adding them.

Padang Cassia is a type of cinnamon (Indonesian or Korintje cinnamon) grown mainly in Indonesia and used widely as a kitchen spice. As a supplement, evidence for health benefits like blood sugar control is limited and mixed, and like other cassia cinnamons it naturally contains coumarin, which can be harmful to the liver in large amounts. Use food amounts freely, but talk to a pharmacist or doctor before taking concentrated cinnamon supplements.

About Ivosidenib

What Is Ivosidenib Used For?

Brand name: Tibsovo

Tibsovo (ivosidenib) is used to treat adults who have a susceptible IDH1 mutation, confirmed by an FDA-approved test. It treats newly diagnosed acute myeloid leukemia (AML) in adults 75 or older, or those who cannot tolerate intensive chemotherapy — either alone or combined with a medicine called azacitidine. It also treats relapsed or refractory AML (AML that has come back or stopped responding to treatment) and relapsed or refractory myelodysplastic syndromes (MDS).

Tibsovo is also approved for adults with previously treated, locally advanced or metastatic cholangiocarcinoma (bile duct cancer that has spread) who carry an IDH1 mutation.
